AI receptionist for clinics: what it does and what stays with your staff
What an AI receptionist handles at a clinic, what the research says about reminders and no-shows, and the HIPAA questions to ask any vendor.

An AI receptionist for clinics answers a practice's calls and messages on the first ring, books, reschedules and confirms appointments in the practice's own scheduling system, and sends the routine follow-ups: reminders, invoices and aftercare emails. Anything clinical, urgent or above a set limit goes to a person on staff.
What does an AI receptionist for clinics do?
It does the repeatable work of the front desk, in the practice's own systems, under rules the practice writes:
- Answers every call and message. Phone, text, web chat, email and WhatsApp, including evenings, weekends and lunch hours.
- Answers routine questions. Hours, location, services and preparation instructions the clinic has approved.
- Books, reschedules and cancels. Directly in the scheduling system, matched to the right service, practitioner and visit length.
- Confirms and reminds. And offers a new time when a patient replies that they cannot make it.
- Handles admin after the visit. The invoice with a payment link and the aftercare email the clinic wrote.
- Hands off what is sensitive. Clinical questions, urgent calls and exceptions go straight to staff, with the whole conversation attached.
Voicemail records a message and a typical answering service passes it on. An AI receptionist answers, acts in the schedule and follows up.
| Task | Voicemail | Typical answering service | Online booking page | AI receptionist |
|---|---|---|---|---|
| Picks up a Sunday evening call | Records it | Takes a message | Not a phone channel | Answers and handles it |
| Books into the real calendar | No | Sometimes, by hand | Yes, for simple bookings | Yes, by service and practitioner |
| Answers "Do I need to fast before my visit?" | No | Reads a script, if one exists | No | Gives the clinic's written instructions, or routes to staff |
| Moves a visit when the patient cancels by text | No | No | Only if the patient logs in | Yes, and offers the freed slot to others |
| Sends the invoice and aftercare email | No | No | No | Yes, under the clinic's rules |
Do appointment reminders actually reduce no-shows?
Reminders reduce no-shows, but they do not remove them. A Cochrane review of eight randomized trials with 6,615 participants (2013) found attendance of 78.6 percent with text message reminders against 67.8 percent with no reminder.
Many practices still see the problem growing. In an MGMA poll from August 2026, 32 percent of medical groups reported higher no-show rates so far in 2026 than in 2025.
So a one-way reminder only gets you part of the way. If "I can't make it Thursday" leads to a new time on the spot, and the freed slot goes to someone on the waitlist, the gap closes. That reply-and-rebook loop is front desk work, which is why it belongs with the receptionist, human or AI.
What should you ask an AI receptionist vendor about HIPAA?
Ask about HIPAA before price or features, because an AI receptionist will hear names, dates of birth and reasons for visits.
The U.S. Department of Health and Human Services guidance on HIPAA and cloud computing says that a cloud service provider that creates, receives, maintains or transmits electronic protected health information for a covered entity is a business associate, even if it stores only encrypted data without the key. The two parties must sign a business associate agreement (BAA).
Whether HIPAA applies to your particular practice, for example a cash-pay med spa, is a question for your compliance adviser. These questions are worth asking any vendor either way, including us:
- Will you sign a BAA, and which subcontractors that touch patient data are covered by their own agreements?
- Where are call recordings and transcripts stored, for how long, and who can access them?
- What access controls and audit logs do we get, and can we export them?
Get the answers in writing, and be wary of any vendor that cannot give a clear yes or no on the BAA.
Frequently asked questions
Can an AI receptionist give medical advice?
An AI receptionist should not give medical advice. It should handle scheduling, questions about your services and admin, and route anything clinical or urgent to your staff right away.
Will an AI receptionist replace my front desk staff?
An AI receptionist takes the repetitive calls and messages off the front desk. Your staff keep the work that needs judgment and care: patients in the room, clinical questions, exceptions and approvals.
Does an AI receptionist work with my scheduling system?
It is only useful if it books into the calendar you already use. Aurora connects to your scheduling system, practice management software and CRM, so ask about your specific system during the demo.
